Fingerprint Clearance Card Information

To obtain an Arizona Fingerprint Clearance Card, Designated Representatives may apply online at https://psp.azdps.gov/services/fccFormTriage For questions about the process, you may contact the Department of Public Safety (DPS) by calling 602.223.2279. DPS office hours are Monday through Friday, 8:00 am to 5:00 pm.

Applicants must provide the Board with a copy of their card. The Board will not receive a copy of the card from DPS.
